Gynecomastia Surgery vs Male Chest Liposuction

A full male chest has two possible causes: firm gland tissue or soft fat. They look similar under a shirt but need different procedures. Gynecomastia surgery removes gland. Chest liposuction removes fat. Many men need a combination of both. This guide explains how doctors tell the two apart with ultrasound, and what each fix involves.

Gland and Fat Are Different Tissues

True gynecomastia is a benign overgrowth of glandular breast tissue, driven by the balance between hormones. It appears in about half of boys during puberty and often settles on its own. Pseudogynecomastia, the fatty type, is simply fat stored on the chest and usually comes with overall weight gain. The distinction also matters medically: new gland growth in an adult can signal an underlying condition, so it deserves a checkup, not just surgery.

Ultrasound Tells Them Apart

A physical exam gives the first clue. A firm, rubbery disc under the nipple points to gland. Soft, even fullness that pinches like belly fat points to fat. Ultrasound then confirms the answer, showing how much gland sits under the nipple and how thick the fat layer is across the chest. Blood tests for hormone levels are added when the cause of new gland growth is unclear.

What Chest Liposuction Alone Can Fix

The fatty type responds well to male chest liposuction alone. Thin cannulas remove the fat layer through hidden openings, flattening the chest and letting skin redrape. At TheLINE Plastic Surgery in Seoul, a chest case runs about 2 hours under sedation with small incisions hidden in natural lines. Liposuction alone will not remove a dense gland disc, however. Suction cannot pull firm gland tissue through a cannula, which is why some men see a persistent puffy nipple after fat-only treatment elsewhere.

When Gland Removal Is Added

Glandular cases need excision. Surgeons either remove the disc through a small incision at the edge of the areola or use a pull-through technique that extracts gland through the same tiny opening used for liposuction. Published series describe combined liposuction plus gland removal as the standard approach, with good chest contour and a low complication rate. Single-incision, no-drain versions have also been reported, keeping scars minimal.

Recovery and Chest Shape

Recovery is similar for both: a compression vest worn full-time for 3 days then part-time, stitches out around day 7, and desk work in about a week. Swelling hides the final line at first, and chest skin generally retracts well in younger men. TheLINE maps gland and fat with ultrasound before recommending either plan, drawing on clinic-recorded liposuction experience of 80,000+ cases since 2007. Browse before and after photos to judge results, review the week-by-week recovery guide, and consult a board-certified specialist to confirm which type you have.

FAQ

Can liposuction alone cure gynecomastia?

Liposuction alone treats only the fatty type. True gynecomastia contains a firm gland disc that suction cannot remove, so glandular cases need excision as well. An ultrasound exam before surgery shows which type you have.

How do I know if my chest is gland or fat?

A rubbery lump directly behind the nipple suggests gland, while soft fullness across the chest suggests fat. Only ultrasound confirms the mix reliably. Clinics such as TheLINE scan the chest during consultation before proposing a plan.

Does gynecomastia come back after surgery?

Removed gland tissue does not regrow, so recurrence is uncommon. Chest fat can still return with major weight gain, and hormone-affecting drugs or steroids can stimulate remaining tissue. Stable habits protect the result.

What scar does gynecomastia surgery leave?

Gland excision typically leaves a fine line along the lower edge of the areola, where it fades into the color change. Pull-through techniques use only the micro openings made for liposuction. Both heal discreetly in most men.

Is male chest liposuction in Korea suitable for foreign patients?

Male chest liposuction in Korea works well for short medical trips, since discharge is same-day and stitches come out around day 7. TheLINE is a Ministry of Health and Welfare-registered clinic for international patients with interpreter support.
